Participant Release and Knowledge of Agreement
The following are a series of statements about rights and expectations. Please review these and confirm your understanding of these rights and expectations to complete this application.
I understand that I am not obligated to perform nor participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time during my training sessions. I understand that should I feel lightheaded, faint, dizzy, nauseated, or experience pain or discomfort, I am to stop the activity and inform my Personal Trainer.
I understand the results of any fitness program cannot be guaranteed and my progress depends on my effort and cooperation in and outside of the sessions.
3. Return on Investment
I understand that all Personal Training rates are based on 55 minute sessions and should I arrive late, there is no guarantee I will receive the full session with my trainer. In return, if my Personal Trainer is late for a session, I will receive the full session time.
4. Transfers and Refunds
I understand that all Personal Training sessions are non-transferable and non-refundable. I also understand the all Personal Training sessions may only be redeemed during an active membership.
5. Cancellation and Rescheduling
I understand that I am required to provide 24 hours notice when cancelling an appointment. No charge will be levied should I cancel with MORE than 24 hours notice given. Should I cancel a session with LESS than 24 hours prior notice, I will be charged in full for that session. I understand that the Reh-Fit Centre recommends that all cancelled sessions be rescheduled as soon as possible to ensure consistency and fitness progress. I understand all cancellations and re-scheduling be made by contacting my Personal Trainer directly.
6. Touch Training
I understand that during a personal training session, my Personal Trainer may have to use Touch Training to correct alignment and/or to focus my concentration on a particular muscle area to be targeted. If I feel uncomfortable or experience any type of discomfort with Touch Training, I will immediately request that it be discontinued.
7. Replacement Trainers
I understand that should my Personal Trainer become ill or is away on holidays, another Personal Trainer may be assigned to me so that my fitness progress does not suffer. I also understand that in the event that my Personal Trainer is no longer employed by the Reh-Fit Centre a suitable Personal Trainer will be re-assigned to oversee my program and workout sessions.
8. Health Information Acknowledgement
I have answered all questions truthfully and to the best of my knowledge. I have not knowingly withheld any information regarding my health. I understand that it is my responsibility to update the Reh-Fit Centre staff about any changes in health status that could affect my ability to safely participate at the Reh-Fit Centre.
I understand that the Reh-Fit Centre photographs many of their client events/sessions and I will always be asked first to provide written approval for them to use these pictures/videos for promotional purposes.
