To be eligible for the program, you need to meet the following criteria:
- Woman*
- Age ≥ 30
- Framingham Risk Score ≥ 10%
- Not currently taking high blood pressure medication
- No diagnosis of cardiovascular disease
*Our program accepts all people who identify as women, including Two Spirit, cisgender, transgender, and non-binary

Step 2
Her Heart Her Way will match you with a mentor to support you every step of the way.
Step 3
Her Heart Her Way begins with assessment, support, and guidance over 12-months through in-person and virtual coaching.

What is Her Heart Her Way?
Her Heart Her Way is a primary prevention program for women at risk of developing cardiovascular disease in the next 10 years. It starts with a referral from a primary care practitioner to the program. Eligible patients are then matched with a Lifestyle Behavioural Mentor who will work together with the patient to identify goals and work towards overcoming barriers in order to live a healthy lifestyle. The program is delivered virtually over the phone or by computer. Physiological and behavioural measurements are taken before, during, and after participation. Periodic reporting back to the referring primary care provider takes place throughout the program.
This program is jointly offered through the Reh-Fit Centre and the Winnipeg Regional Health Authority and is free of charge for participants.
Is Her Heart Her Way right for me?
Her Heart Her Way is intended for women with increased risk of cardiovascular disease and are willing to make a change.
Risks of cardiovascular disease are high cholesterol, diabetes, high blood pressure and family history. Lifestyle risk factors are unhealthy diet, physical inactivity, unhealthy weight, smoking, stress, excessive alcohol & drug abuse.
How do I join Her Heart Her Way?
Your primary care provider (doctor or nurse practitioner) will determine if you are eligible for the program based on your blood test results and your risk of cardiovascular disease (Framingham Risk Score).
